Level by level
Hollow Heart goes up one level each time you pick it again on a level-up (a chest can also give it a level), up to level 5. Nothing else levels it up: other passives never do. The table shows what it gives in all at each level.

Evolves
Hollow Heart can be at any level: only the weapon marked MAX has to be maxed.
Other things that change Max HP
These do not level up Hollow Heart. Each one adds its own bonus to Max HP, and the game adds every bonus together. For example, Hollow Heart at level 5 (+100%) + Max Health at rank 3 (+30%) = +130% Max HP.
