Level by level
Empty Tome goes up one level each time you pick it again on a level-up (a chest can also give it a level), up to level 5. Nothing else levels it up: other passives never do. The table shows what it gives in all at each level.

Empty Tome can be at any level: only the weapon marked MAX has to be maxed.
Other things that change Cooldown
These do not level up Empty Tome. Each one adds its own bonus to Cooldown, and the game adds every bonus together. For example, Empty Tome at level 5 (-40%) + Cooldown at rank 2 (-5%) + Blox Paladin (-10%) = -55% Cooldown. The game never lets the cooldown go below 40%.
